The National Library of the Philippines (NLP) has released 𝐑𝐞𝐯𝐢𝐬𝐢𝐨𝐧 𝟐 of the 𝗖𝗼𝗽𝘆𝗿𝗶𝗴𝗵𝘁 𝗥𝗲𝗴𝗶𝘀𝘁𝗿𝗮𝘁𝗶𝗼𝗻 and 𝗗𝗲𝗽𝗼𝘀𝗶𝘁 𝗔𝗽𝗽𝗹𝗶𝗰𝗮𝘁𝗶𝗼𝗻 𝗙𝗼𝗿𝗺. This updated version is now the official form required for all new applications.

Implementation Timeline for Copyright Form Revision 2:

𝟭𝟭 𝗔𝘂𝗴𝘂𝘀𝘁 – 𝟵 𝗦𝗲𝗽𝘁𝗲𝗺𝗯𝗲𝗿 𝟮𝟬𝟮𝟲 (𝗧𝗿𝗮𝗻𝘀𝗶𝘁𝗶𝗼𝗻 𝗣𝗲𝗿𝗶𝗼𝗱): Both the Original Form and Revision 1 remain valid and accepted.

𝟭𝟬 𝗦𝗲𝗽𝘁𝗲𝗺𝗯𝗲𝗿 𝟮𝟬𝟮𝟲 𝗢𝗻𝘄𝗮𝗿𝗱: Revision 2 is strictly required.

𝗡𝗼𝘁𝗲: Applications submitted on outdated forms after 𝟵 𝗦𝗲𝗽𝘁𝗲𝗺𝗯𝗲𝗿 will be returned, and applicants will be asked to resubmit using Revision 2.
