It may not be a lack of discipline.
The problem is often the intensity of the beginning.
You attempt to change your routine, habits, sleep, diet, work, and mindset at the same time. It feels powerful for several days, but the plan depends on maintaining maximum effort indefinitely.
When that intensity becomes impossible to sustain, you stop and blame yourself.
A better strategy is to begin at a level that feels almost too easy:
Five minutes.
One page.
One important task.
One manageable action.
You can increase the effort after the behaviour becomes stable.
Do not build your plan around your most motivated day. Build it around an ordinary day.
Consistency is not what you can do once. It is what you can repeat.
