Is It Truly Safe — or Have We Simply Been Fortunate So Far?

One of the most important responsibilities in HSE leadership is distinguishing between a task that has been performed successfully in the past and a task that is genuinely safe under today’s conditions.

The same activity may carry very different levels of risk depending on the people involved, equipment being used, work environment, simultaneous operations, and changing site conditions.

Yet we still hear:

> “We have always done it this way.”
“We have never had an incident.”
“The risk assessment is already approved.”

None of these statements confirms that the risk is adequately controlled.

The absence of an incident is not evidence of the absence of risk.

A strong safety culture is not measured solely by the number of procedures issued, audits completed, or incident-free days achieved. It is reflected in the quality of decisions made at the point of work—especially when conditions change, assumptions are challenged, or operational pressure competes with safety.

Risk assessments, method statements, PTW systems and procedures establish the framework. But effective HSE leadership requires us to continuously verify that the controls identified on paper are actually present, understood and effective in the field.

Before starting—or continuing—an activity, we should ask:

Is it safe for the people performing the work?
Have conditions changed since the risk assessment was prepared?
Are the identified controls actually in place?
Are workers competent and properly briefed?
If something goes wrong, are we prepared to respond?

The objective should never be simply to say, “We completed the work without an incident.”

The objective should be to understand why the work was completed safely—and whether we can reliably achieve the same outcome tomorrow.

Compliance establishes the standard. Leadership creates accountability. Culture determines what happens when no one is watching.

In an electrical emergency:
🔹 Do NOT touch the person or equipment directly.
🔹 Isolate the power supply immediately, if safe to do so.
🔹 Raise the alarm and call emergency services.
🔹 Provide CPR/First Aid only when the electrical source is confirmed isolated and it is safe to approach.

Remember: Isolate first. Rescue safely. Save lives.

SAFETY BARRIERS — One Barrier Is Never Enough!

Incidents rarely happen because of one single failure. They often occur when multiple safety barriers fail or are bypassed at the same time.

The Swiss Cheese Model reminds us that every control can have weaknesses. That’s why we need multiple layers of protection:

🔧 Engineering Controls — Guarding, isolation/LOTO, ventilation, safety devices
📋 Administrative Controls — PTW, JSA/RA, procedures, training & inspections
👷 Behavioral Controls — Following procedures, correct PPE use, reporting unsafe conditions
🛡️ Mitigating Barriers — Emergency shutdown, fire protection, evacuation & emergency response

When the weaknesses in these barriers line up, a hazard can pass through and become an incident, potentially resulting in injury, equipment damage, environmental harm, or financial loss.

Never rely on a single control. Strong safety comes from multiple independent barriers working together.

🦺 Safety is a mindset, not just a rule.

Real safety culture begins when people stop wearing PPE simply because “it’s required” and start thinking:

“I want to go home safely — and I want my colleagues to go home safely too.”

Procedures, permits and PPE are essential, but a strong safety culture is built when safe behaviour becomes instinctive.

This video is a powerful reminder of how quickly an industrial operation can turn dangerous. Hot materials, moving equipment, stored energy, pressure, and unexpected equipment behavior can lead to serious incidents within seconds.

✅ Always follow the approved SOP / Method Statement
✅ Maintain a safe exclusion zone
✅ Never stand in the line of fire
✅ Ensure guards and safety devices are in place
✅ Identify stored-energy hazards before starting work
✅ Stop the job immediately if conditions become unsafe

Remember: If you are in the line of fire, one unexpected movement can be enough.

⚠️ ISOLATION = ZERO ENERGY BEFORE WORK! 🔒

Before maintenance starts, simply switching equipment OFF is not enough. All hazardous energy must be properly isolated, locked, and verified.

⚡ Electrical Isolation – Disconnect and lock the power source.
⚙️ Mechanical Isolation – Prevent movement or rotation.
🔩 Positive Isolation – Physically separate the energy source, e.g., blind/spade.
🔧 Process Isolation – Isolate valves and drain/vent the system.
💨 Hydraulic/Pneumatic Isolation – Release stored pressure and prevent re-pressurization.
