⚠️❗SAFETY FLASH❗⚠️

We received a report yesterday that there was a diving operations incident.

A diver was conducting water jet cleaning at the vessel's rope guard area when the vessel activated its propeller.

The diver and his umbilical was caught in its initial spin , spinning him round at least three times before he was thrown out of the spin. He fortunately did not incur life threatening injuries.

The Dive team immediately executed the emergency response plan and the diver was evacuated to the hospital.

Investigations are underway. However it is clear this was a breach of protocol by the vessel.

All diving supervisors are encouraged to ensure all LOTO procedures are followed accordingly. Ensure to check in every 4 hours on longer job to ensure the vessel is compliant to your LOTO.
