CMS' release of QSO-23-16-Hospital Memo is a welcomed reminder in the coordination of services between providers. The QSO Memo was released this week on June 6th and reminds hospitals, survey agencies and accreditation agencies of the need to provide the acute care condition along with the comorbid conditions on patients discharging from the hospital. The memo specifically references inclusion of serious mental illness diagnoses, substance abuse disorders, supervision levesl during hospital stay, medications, skin condition, CPAP/BiPAP levels, advance care plan decisions and communication about the care needs at admission. With the changing environment, this is a welcomed reminder on the communication needs to allow post acute providers the opportunity to better support patient care and prevent unnecessary hospital visits.

I have had this DNS-CT certification on my to do list for awhile and decided to invest in myself to accomplishing this goal in 2023! Proud to say I met this goal!
