More than 80 years later, the year 1945 still matters in VA disability evaluation.

At the end of World War II, the Veterans Administration released the 1945 Schedule for Rating Disabilities. While the schedule has been modified and updated many times since, VA identifies that 1945 framework as the basis of the system still used today.

It’s an interesting example of how a decades-old framework continues to evolve alongside changes in medicine, terminology, and our understanding of disability.

A medical opinion and a VA determination are not the same thing.

A medical opinion addresses the clinical question: what does the evidence medically support based on the history, records, findings, and reasoning?

What it does not do is decide the final VA outcome, assign a rating, or guarantee a result.

That distinction matters.

The clinician explains the medicine.
The adjudicator makes the determination.

A treatment gap tells us that care was not documented during a particular period.

What it does NOT tell us, by itself, is why.

Symptoms may have been managed independently, normalized, tolerated, deprioritized, or simply not brought into a clinical encounter. In other cases, the absence of treatment may be medically meaningful.

The point is not to dismiss the gap - it is to avoid assigning an explanation without supporting context.

In medical record review, the distinction between what is documented and what is inferred matters.

As a National Aviation Day reminder following August 19, here’s a milestone from the early history of aviation medicine.

In 1918, the U.S. Army established its first course for flight surgeons - and the very first class included just 3 physicians.

It was an early sign of something important: as aviation advanced, medicine had to advance with it.

The demands of flight created a need for clinicians who could understand the unique medical considerations of aviators - a concept that helped shape the field of aviation medicine over time.

Over time, chronic pain may also affect how someone sleeps, moves, works, participates in activities, and experiences daily life.

That is why functional history can matter when reviewing a condition over time. A diagnosis identifies the condition - but the broader record may help show how that condition is actually affecting the individual.

Looking beyond a single visit can provide important clinical context.
