Buying the right home is a checklist. Operations is a system.

One of the biggest mistakes you can make is buying a home before understanding what it will take to actually operate it.

Before you purchase, you need to understand:

- Is the zoning appropriate?
- Do distance rules apply?
- What are the state and local licensing requirements?
- What fire and life safety measures will be required?
- Will you need things like a sprinkler system, wheelchair ramp, widened door frames, or grab bars?

Those costs can add up quickly.

You don't want them to be a surprise. You want to understand them before you buy and have them in your underwriting.

Isolation is expensive. In older adults it shows up as ER visits, as new prescriptions, as memory loss that didn't have to happen. Company, a warm meal, and someone who notices the small changes aren't extras - they're care. It's the part that never makes the spreadsheet and matters most.

Not all confusion is dementia - some of it is a 911 call. Three signs it's an emergency: it came on fast (sharp yesterday, lost today), it comes with a body change (one-sided weakness, facial droop, slurred speech), or it flickers through the day (clear one hour, gone the next). The 2026 Alzheimer's numbers landed this week, and one of them stopped me as a physician. 7.4 million Americans over 65 are living with Alzheimer's right now, on track to nearly double to 13.8 million by 2050.

But the number underneath it is the one that stays with me. Nearly 13 million people are providing unpaid care for someone with dementia - 19 billion hours last year, valued at 446 billion dollars. That's not a statistic. That's a daughter who quit her job. A spouse who hasn't slept properly in two years.

And here's the hard truth that sits under this whole industry: every one of those families becomes a customer the day they can no longer do it at home.

For operators, memory care is the highest-acuity, highest-revenue-per-resident thing you will ever run - and the hardest to do well. Getting it right isn't a nice-to-have. It's the whole job.

2 β€” Healthcare power of attorney: names who makes medical decisions when your parent can't speak for themselves.

3 β€” Living will / advance directive: puts their wishes about life support and end-of-life care in writing, so no one has to guess.

4 β€” HIPAA authorization: without it, doctors can legally refuse to talk to you about your own parent.

5 β€” Current medication list: every drug, dose, and pharmacy, on one page. The single most useful thing in any emergency room.

6 β€” Insurance + ID: Medicare, supplement, long-term care policy, and a photo ID, copied and shared with the family.

7 β€” A list of doctors and key contacts: names, numbers, and who does what, so it isn't in one person's head.

What if the seller's goals could help you structure the deal?

The sellers wanted consistent monthly income instead of a lump-sum payment, so seller financing became an option.

The deal came together at $740K with $45K down, giving us our fifth lease-to-operator residential assisted living home.

Sometimes understanding what the seller actually wants can open up a different way to structure the deal.
