Today, April 16, 2026, is National Healthcare Decisions Day!

This day encourages individuals to reflect on and document their healthcare preferences, ensuring that their wishes are respected in times of need. Here at GCP, we understand the importance of these conversations and are here to support families in navigating these critical decisions.​

Let's take this opportunity to initiate discussions with our loved ones about their healthcare choices and advance directives. By doing so, we honor their autonomy and ensure peace of mind for everyone involved.​

For this Giving Tuesday, we'd like to highlight two local nonprofit organizations in which we participate here in NWO:

First, we have the Stroke Life Center located in downtown Toledo. This organization was founded by stroke survivors for stroke survivors. They provide extended physical and occupational therapies, support groups, and socialization opportunities to stroke survivors- free of charge :)

Next, we have Connecting People & Places. This is an organization helping folks with developmental disabilities to become involved in their community. They run a day program in which the individuals complete tasks and volunteer to serve their communities and give back.

Both of these organizations are doing very important work here in NWO and on this Giving Tuesday, we ask that you check them out and donate to their causes if you feel called to do so!

Two years ago, I met a wonderful lady who was living in her own senior apartment next to her best friend. Her family was looking at nursing home options at the time but she was adamant that she didn't want that.

Through careful coordination of services, advocating for her needs, and closing gaps in care, she’s been able to stay safely in her home! Her family’s stress is reduced, and they’ve avoided the extra costs a facility would have brought.

Today, she’s happy, comfortable, and thriving... living life on her own terms.

This is what care management is all about: supporting independence, giving families peace of mind, and keeping people where they feel at home.

As a social worker, I have had the privilege of hearing many stories behind the uniform—stories of courage, sacrifice, pain, strength, resilience, and humanity.

Today, we honor every veteran who served this country, not just on the battlefield but long after—when the uniform comes off and the real challenges begin. The battles with trauma, housing, benefits, identity, health, and healing are just as real, and too often invisible.

To our veterans:
Thank you for your service, your sacrifice, and your stories.
Thank you for trusting us—social workers, advocates, and helpers—to walk alongside you.
You deserve dignity, support, and a system that shows up for you every single day.

May we continue fighting for the resources, the care, and the respect you have earned.
Your lives matter. Your wellbeing matters. And we see you.

🏡 Home Care vs. Care Management: What’s the Difference?

Many families use the terms home care and care management interchangeably but they’re actually very different types of support.

💛Home Care
Provides hands-on help with daily activities — things like bathing, meal prep, light housekeeping, medication reminders, and companionship.

💛Care Management
Provides professional oversight, advocacy, and coordination of care. A Geriatric Care Manager assesses needs, creates a care plan, connects families with the right resources, and ensures every piece of the care puzzle fits together.

Together, home care and care management create a complete circle of support by helping older adults stay safe, independent, and well cared for, while easing the stress on families.

Yesterday, we hopped on a charter bus from Toledo to Birch Run and Frankenmuth for a fun-filled day of holiday shopping, food, and laughter. 🚌 🛍️ 🍗

We love partnering with other amazing companies in NWO to celebrate the incredible social workers, case managers, and nurses we work with who make a difference every single day! 💛
