Supermarket Fined $196,251 After Worker Loses Four Fingers in a Meat Grinder
OSHA cited a supermarket chain in Georgia with a willful violation for bypassing the machine's safety guards after a co-worker accidentally activated a commercial grinder's foot pedal while a worker was cleaning it, amputating four of his fingers — the employer also received a serious violation for having no hazardous energy control program whatsoever. That the injury was caused by a missing lockout/tagout procedure — one of OSHA's most cited and most trainable standards — makes it particularly difficult to defend.

OSHA Is Now Issuing One Citation Per Worker, Not One Per Violation.
Under OSHA's instance-by-instance citation policy, the agency can issue a separate citation for each individual worker exposed to a hazard rather than grouping them into a single violation — meaning five workers without fall protection on a construction site generates five serious citations at up to $16,550 each, and for willful violations, five instances at $165,514 each would total over $827,000 from a single observed condition. Most employers calculate their worst-case penalty exposure using the per-violation rate; very few have modelled what a per-worker calculation produces.
