Your safety numbers improved for a few years — then flatlined. Same effort, no movement.
That plateau isn’t a knowledge problem. It’s the point where programs, PPE, and training have done everything they can do, and the next gain lives somewhere they can’t reach: whether a supervisor actually stops the line, whether an operator reports the near-miss or swallows it.
Worth knowing — a falling recordable rate can hide a flat serious-injury exposure. The number looks better while the catastrophic risk stands still.
