Mercy Health — St. Rita’s Medical Center has been recognized with the inaugural Ohio Stroke Award for Transitions of Care by the Ohio Stroke Collaborative, a statewide organization dedicated to improving stroke care across Ohio.

This recognition highlights St. Rita’s commitment to helping stroke patients and their families successfully transition from the hospital back home.

Through a research study conducted by our inpatient rehabilitation team, we discovered that many stroke patients and caregivers did not feel fully prepared for life after discharge. In response, our team implemented the PATHs tool, an innovative assessment that helps identify caregiver needs and provides personalized education, resources and support before patients leave the hospital.

By involving nurses, therapists, physicians and caregivers in the discharge planning process, we are helping patients feel more confident and supported during recovery.

The impact is already being seen. More stroke patients are successfully returning home after rehabilitation, and the program’s findings have been shared nationally at the American Rehabilitation Nurses Association Conference, helping advance stroke care beyond our community.

We're also looking ahead. St. Rita’s is working to launch the PATH App, which will provide ongoing support, education and resources to patients and caregivers for up to six months after discharge.

This award reflects the dedication of our multidisciplinary care teams and their ongoing commitment to innovation, research and patient-centered care. Congratulations to everyone at Mercy Health — St. Rita’s who helped make this achievement possible!
