If COPD and shortness of breath while lying flat are being coded on the MDS, make sure your team knows what the record needs to support.

This is an area receiving significant audit attention, and simply documenting that the head of the bed was elevated is not enough on its own.

The medical record should support what the resident experienced during the 7-day lookback period, including observed symptoms, staff-reported symptoms, or documentation that the resident avoids lying flat because of shortness of breath and what interventions were used.

The way falls with major injury are measured is changing💡 Beginning in January 2027, the Five-Star measure will transition to a hybrid model incorporating MDS and claims data. That makes what happens after a fall even more important for facilities to understand now.

Isolation on Section O is going to be audited. Are you sure your documentation supports it? ✅

This is one of those MDS topics where the diagnosis alone does not determine whether isolation can be coded.

All four requirements have to be met, and the medical record has to clearly support what was ordered, when isolation began and ended, the resident’s room status, and how services were provided during the isolation period.

And there are important exclusions that can change the answer entirely.

UTI. Wounds. Pneumonia. C. diff. The details matter, and assumptions can create coding and compliance risk.
