Active diagnoses play an important role in the MDS and PDPM.

Diagnosis codes are used to identify the resident’s primary diagnosis in I0020B, as well as additional diagnoses and comorbidities that can factor into the SLP, NTA, and Nursing components of PDPM. A few important reminders:

• MDS nurses are responsible for identifying active diagnoses and conditions for Section I.
• Supporting documentation can include care, monitoring, and interventions
• Functional limitations and nursing monitoring can also support an active diagnosis.
• Historical or inactive diagnoses that are not impacting care should not be marked as active on the MDS.

Accurate documentation matters, not only for compliance, but for ensuring the MDS accurately reflects the resident’s current condition and care needs.
