Please submit info below and we will contact you for the pick up or drop off if required. 1 box of single use mask and a bottle of hand sanitizer per household. Please don't visit office until we call you.
We are also distributing in Pittsburgh and Lancaster area. You can fill up the same form if you resides in those areas and we will contact you with time and location for pick up.
