I sat across from a clinic office manager a while back who told me their HIPAA risk assessment was "on the list." That list had been sitting untouched for two years.

Nobody in that practice was careless. They were just busy. Patient care came first, and the server room came second, third, or not at all.

Here is the part that stuck with me. The breach that eventually hits a practice like that is rarely dramatic. It is usually one unpatched system or one vendor nobody vetted, sitting quietly until it is not quiet anymore.

We walked that practice through three things. A real risk assessment instead of a checkbox. Encryption on every device touching patient records. A written incident response plan so the first 24 hours after a problem are a checklist, not a scramble.
