An EOB can look like a wall of numbers at first.

But most of the time, you only need to understand four numbers to know what happened:

📄Billed amount - What you charged for the service.

📄Allowed amount- What the payer agreed to under your contract.

📄Paid amount - What the insurance company actually sent.

📄Patient responsibility - What the client owes through copay, coinsurance, or deductible.

The important check:

Paid amount + patient responsibility = allowed amount

When those numbers do not line up, something needs a second look.

Setting up a private practice is rarely hard because the tasks themselves are complicated. It's hard because the pieces live in different places. Your state board handles licensing. The IRS handles your EIN. CAQH and the insurance panels handle credentialing. HIPAA sits over all of it. Get the order wrong and you end up redoing paperwork, or waiting three months on credentialing you could have started early.

So we wrote down the full setup checklist for mental health practices, grouped into four phases and put in the order we'd actually work through it with a clinician: your legal and financial base, your provider IDs and insurance, your clinical systems and compliance, then the work of getting clients in the door.

It also covers the part most people miss: the small inconsistencies (a name that doesn't match across your NPI, CAQH, and tax records) that quietly add weeks to credentialing.
