Pickup day can feel like the finish line, but for many families, it's really the beginning of the next phase of care.

Before your loved one leaves the hospital, rehab center, or care facility, make sure you know what medications have changed, when follow-up appointments are scheduled, and what support they'll need once they get home.

It's also important to think beyond the medical instructions. Who will prepare meals? How will they get to appointments? Is the home safe to move around in? Will someone be there if they need help?

Loose cords and throw rugs cause more falls than most families realize.

After years working inside hospitals and long-term care facilities, we noticed the same pattern over and over. It wasn't a big renovation that kept seniors safe, it was small, boring fixes: taping down cords, swapping throw rugs for non-slip mats, and moving the pet bowl out of the hallway.

In just under two years, we’ve had the privilege of serving families across Long Island while experiencing incredible growth. Thanks to the trust of our clients, referral partners, and dedicated companions, we’ve reached a point where it’s time to build the next level of leadership within our organization.
