"We'll sort it out once we're home." It's one of the most common things families say, and one of the most stressful plans to rely on.

Discharge day often comes faster than expected. And the hospital's job ends at the door.

What families often don't realise until they're in it:

- Prescriptions need to be filled before the ride home, not after
- The house may not be set up for safe movement yet
- Follow-up appointments need to be booked before discharge, not days later
- Someone needs to be there the first night, with a plan, not just good intentions

The 48 hours before discharge are some of the most useful hours you have. Use them.

When something urgent happens, you want to find information in 30 seconds, not 30 minutes.

A straightforward home document kit takes less than an hour to put together and can save a lot of confusion when it matters most.

Here's what to include:

- Full name, date of birth and insurance information
- Current medications, including name, dose, prescribing doctor and pharmacy
- Primary care doctor and any specialists, with phone numbers
- Known allergies and current diagnoses, in plain language
- Emergency contacts in order of priority
- Advance directive or healthcare proxy information, if available
- Location of important documents, such as insurance cards, Medicare info and legal paperwork

Keep one copy at home in a visible, consistent spot. Keep a second copy in a bag that travels with your parent.

This is one of the most straightforward things you can do today. It costs nothing and helps everyone.

What to pay attention to instead:

- How staff greet residents they pass in the hallway, or whether they do at all
- How quickly call bells are answered and whether staff seem rushed
- Whether residents look engaged or simply parked in common areas
- The smell and cleanliness of resident rooms, not just shared spaces
- How staff respond when you ask about increasing care needs

A well-run facility holds up to those questions without hesitation.

Now the calendar is yours, the worry is yours and somehow the guilt is yours too, even when you're doing everything right.

Summer heat is one of the most overlooked risks for seniors at home.

On Long Island, July and August can push indoor temperatures well above comfortable levels, especially in rooms without AC or good airflow. Seniors feel heat differently, and by the time thirst kicks in, dehydration may already be setting in.

A few simple adjustments can make a real difference:

- Keep a water bottle visible on the kitchen counter, not just in the fridge
- Close blinds on sun-facing windows by mid-morning
- Check that fans are actually moving air, not just running
- Identify the coolest room in the house and make it usable
- Plan any outdoor errands before 10am or after 5pm

If you're managing this from a distance, a regular check-in call at midday is one of the easiest things you can add this season.
