7 Charting Mistakes That Can Create Problems for Nurses

Your shift was busy. Medications were given, changes in condition were reported, reassessments were completed—and now your documentation needs to tell the story clearly.

Before signing your next note, watch for these common charting mistakes:

1️⃣ Charting care before it happens
Document assessments, medications, and interventions after they are completed. Plans can change, and the record should reflect what actually occurred.

2️⃣ Leaving out the patient’s response
“Provider notified” is not always enough. Include what happened next: symptoms, reassessment findings, new instructions, and any pending follow-up.

3️⃣ Using labels instead of observations
Avoid vague statements like “patient was difficult.” Document specific behaviors, statements, refusals, education provided, alternatives offered, and follow-up.

4️⃣ Copying an old note without reviewing it
Copied documentation can carry outdated or incorrect findings forward. Review every field and confirm you are documenting in the correct patient’s chart.

5️⃣ Leaving out critical communication
When escalating a concern, document the change in condition, who was contacted, when contact occurred, what was communicated, instructions received, and what you did next.

6️⃣ Making a late entry look on time
Use your EHR’s approved late-entry or addendum process. Clearly distinguish when the care occurred from when it was documented.

7️⃣ Signing without a final review
Double-check the patient, date, times, medications, auto-filled text, and follow-up needs before signing.

A quick pre-sign check:

Assessment → Action → Response → Communication → Next Step

Accurate charting supports continuity of care, patient safety, and clear communication between clinicians.

Save this for your next shift and share it with a nurse who may find it useful.

Can Nurses Use ChatGPT at Work? What’s Safe vs. Not Safe

ChatGPT can be useful for nurses, but whether it is appropriate at work depends on the task, the information being entered, and your employer’s policy.

Here’s a practical 2026 guide:

🟢 Generally reasonable: learning and preparation

Use AI for general questions that do not include patient information, such as simplifying medical terminology or explaining general clinical concepts.

Always verify the response against current clinical guidance and your facility’s approved resources.

🟡 Pause and check: work-related tasks

Before using AI to draft patient education, summarize policies, or support documentation, confirm that your organization allows it and that the tool is approved.

An employer-approved AI system may have different privacy safeguards than a personal ChatGPT account.

🔴 Keep patient information out of personal or unapproved AI tools

Do not paste charts, handoffs, patient messages, photos, recordings, or other identifiable information into public AI tools.

Removing a patient’s name may not be enough. Details such as age, dates, location, diagnosis, and circumstances may still identify the patient.

🔴 Never rely on AI as your only clinical source

AI can provide inaccurate, incomplete, or outdated information. Verify medication details, procedures, contraindications, and clinical recommendations through approved sources.

🔴 Never document care that did not happen

If AI-assisted charting is permitted, review every line before signing. Confirm that the note accurately reflects your assessment, interventions, communication, and reassessment.

Before using AI, ask:

✅ Is this tool approved for this task?
✅ Could this information identify a patient?
✅ How will I verify the answer?

AI may help save time. Protecting patient privacy, maintaining accurate documentation, and exercising clinical judgment remain the nurse’s responsibility.

Save this for your next shift and share it with your nursing team.

🤖 10 Things Every Nurse Should Know About AI in 2026

AI is becoming part of nursing workflows, but speed should never replace safety, clinical judgment, or accountability.

The American Nurses Association’s 2026 guidance reinforces an important principle: AI should support nursing judgment, not replace it.

Here’s a practical checklist every nurse should keep in mind:

1️⃣ Check workplace policy first
Use only AI tools approved by your organization for clinical work.

2️⃣ Protect patient privacy
Never enter names, MRNs, photos, chart notes, or identifiable patient information into public AI tools.

3️⃣ Use AI for drafts, not final decisions
AI may help organize or explain information, but nurses remain responsible for validating the output.

4️⃣ Verify every clinical claim
Double-check medications, doses, contraindications, procedures, and recommendations using approved clinical resources.

5️⃣ Review AI-assisted documentation carefully
Your chart must accurately reflect what you observed, performed, communicated, and reassessed.

6️⃣ Remember AI may miss context
New symptoms, family concerns, patient preferences, and subtle changes in condition may not be captured.

7️⃣ Question recommendations that don’t fit
If AI output conflicts with your assessment, follow your clinical escalation process.

8️⃣ Watch for bias
Consider language, health literacy, disability, resources, culture, and individual patient preferences.

9️⃣ Be transparent about AI use
Follow your organization’s requirements for reviewing, correcting, and disclosing AI-generated content.

🔟 Keep the human connection
AI can support the conversation, but empathy, observation, communication, and clinical judgment remain human responsibilities.

Before using AI, ask:

✅ Is this tool approved?
✅ Is the information safe to enter?
✅ Have I verified the output?

Save this for your next shift and share it with a nurse navigating AI in healthcare.
