We are seeking a dedicated and compassionate service coordinator (clinical) to join our team. In this role, you will play a vital role in our care coordination process by conducting assessments, identifying members needs, and developing person center service plans. Working as part of an interdisciplinary team, you will collaborate closely with care coordinators to ensure our members receive the comprehensive support they require. This position does require 50% travel and 50% remote office work.

It’s been a while since we last shared an update, and we appreciate you being here with us! As we move through the winter months, food insecurity becomes an even greater challenge for many individuals and families. Higher heating costs, limited work hours, and school breaks can stretch already tight budgets, making it harder to put food on the table. Cold weather also reduces access to transportation and community resources that people rely on year-round.

This season is a reminder of how important it is to look out for one another. Together, we can help ensure our neighbors have access to the nourishment and support they need to stay healthy and warm all winter long.

Today, as we close out the week, I want to take a moment to shine light on an issue that affects far too many families in our communities: food insecurity.

Millions of children, seniors, and hardworking adults face the harsh reality of not knowing where their next meal will come from. It’s more than hunger, it’s the stress of choosing between groceries and rent, between feeding your children and keeping the lights on. It’s a silent struggle, but one that deserves our loudest attention.

Fall break is here! With the kids home all week, the kitchen can start to feel like it’s open 24/7. If you’re looking for ways to keep everyone fed (without losing your sanity), here are a few simple ideas:

Happy Thursday! We’re almost to the weekend, but let’s be real, Thursday is basically the perfect day to treat yourself. Not quite Friday, but definitely a step up from Monday. 😉

So here’s a little food inspo for today:
