It has truly been an honor to serve our community through Radius10 and RadiusCare for more than six years. During that time, we’ve placed exceptional leaders, partnered on meaningful initiatives, and built long-term care coordination that supports caregivers and clients across the Midwest and the nation.

As co-founder and CEO, I couldn’t be more proud of the impact this company and team continue to make.

I’m now stepping into an exciting new opportunity that allows me to serve in a way that brings deep personal and professional fulfillment. While I look forward to this next chapter, RadiusCare will continue supporting our concierge clients through healthcare coordination, led by a dedicated and passionate team deeply connected to the families they serve.

📣We are growing 🌱Now hiring multiple healthcare coordinators!! Our patients deserve the absolute highest of care standards ❤️This position is remote with some planned travel required📣

The Healthcare Coordinator helps to support our mission by providing quality care to our patients and is responsible for integrated patient care coordination with their assigned patients. The purpose of healthcare coordination is stabilization, delaying/preventing deterioration of health status, management of chronic conditions, and/or improved health status of the patients served while helping their caregivers have a balanced quality of life, as well, while caring for their loved one.

MINIMUM QUALIFICATIONS:

• Current Indiana RN or LPN license in good standing. (preferred)
• Current Indiana LSW license with a degree in social work with at least 2 years of experience in healthcare coordination.(preferred)
• Knowledge of basic healthcare coordination processes and best practices.
• Knowledge of medical terminology.
• Familiarity with health care systems, regulations, policies, and functions.
• Understanding of documentation standards.
• Ability to communicate effectively in person, by phone, and in writing.
• Ability to establish and maintain effective working relationships with coworkers and diverse patient populations.
• Ability to competently use computer systems and Microsoft Office.

As many providers in our space know, we’ve had patients experiencing ongoing issues with authorizations for incontinence supplies. Diondra saw the need and took it upon herself to go the extra mile — finding additional resources to help support our patients during this challenging time. 🫶

🌟Help us welcome our newest member to the Radius10 + RadiusCare team!! Diondra Igboegwu, CCHW will be serving as our new Community Health Operations & Development Coordinator!

Diondra is a Certified Community Health Worker originally from Hammond, Indiana who is passionate about community health education, wellness, and advocacy. Diondra earned her bachelor's degree in Health Services Management from Indiana University Indianapolis in 2012.

With over a decade of experience in the healthcare field, including at Indiana University Health. Diondra has extensive experience in maternal healthcare both in hospital settings and within the community. She is one of the creators of the Black Maternal Health ECHO series at Indiana University Health.

Currently pursuing further education in health promotions and wellness, Diondra envisions one day opening a wellness center offering affordable or free services and education to the community. Outside of her professional life, she enjoys being a mother to her three-year-old son and exploring new recipes and restaurants. Welcome Diondra!!🌟
