Registered Nurses - Intake and Assessments

In addition to home health aides, we are seeking Registered Nurses (RNs) to join our intake team and conduct initial client assessments. Our intake nurses are responsible for:

- Conducting in-home evaluations of new clients
- Assessing clients' health conditions and capabilities
- Determining clients' needs for assistance with activities of daily living
- Developing customized care plans for our home health aides to follow
- Managing new client admissions paperwork and documentation
- Building rapport with new clients and their families

Ideal candidates will have:

- An active RN license in Florida
- 1+ years of nursing experience, preferably in geriatrics or home health
- Strong assessment, documentation, and care planning skills
- Empathy, comfort working in clients' homes
- Reliable transportation

Our intake RNs play a crucial role in getting new clients set up with properly matched caregivers. If you are an experienced, caring RN looking for flexibility and the chance to work more autonomously in the field, this could be the perfect opportunity!

Home Health Aides Wanted

Our growing homecare agency seeks caring, compassionate individuals to provide non-medical services and companionship to seniors and disabled adults in their homes throughout Jacksonville & St. Augustine, Florida. As a home health aide with our agency, you will help clients with activities of daily living including:

- Bathing and grooming
- Dressing and changing clothes
- Meal preparation and feeding
- Light housekeeping
- Medication reminders
- Transportation for errands and appointments
- Companionship and conversation

We are looking for reliable candidates who have:

- A high school diploma or GED
- Previous experience as a caregiver, CNA, or home health aide
- Excellent communication and interpersonal skills
- The ability to be empathetic and patient
- A caring and compassionate nature
- The willingness to have a background check
