Healthcare fraud is becoming harder to detect through isolated claims alone.

CMS recently announced that its enforcement actions stopped more than $1.6 billion in potentially improper Medicare laboratory payments, including $732 million tied to the revocation of 157 laboratory providers. CMS specifically described its approach as a technology-powered fraud prevention operation.

That matters.

Because modern program integrity increasingly depends on the ability to detect patterns at scale.

AI and advanced analytics can help identify:
→ Abnormal billing behavior
→ Suspicious provider relationships
→ Shared beneficiaries
→ Geographic inconsistencies
→ Emerging scheme indicators
→ Patterns that would be nearly impossible to spot manually

But the signal still has to go somewhere.

What does the provider actually look like?

Who is behind the organization?

Are the services being delivered?

Do the documentation, clinical evidence and real-world activity support what the data is showing?

That is where integrated investigative capability changes the equation.

CoventBridge doesn't make clients choose between technology and investigation.

We connect advanced detection with experienced investigators who can take the signal further.

Detect → Connect → Investigate → Validate → Act

AI and analytics are transforming healthcare investigations.

But one question still matters:

Who picks up the case after the system finds something?

For some organizations, field investigation is outsourced only when a case reaches a certain threshold.

That model can work.

But it is different from having employee investigators integrated into the investigative operation every day.

A connected investigative team can work directly across:

→ Analytics
→ Desk investigations
→ Medical review
→ Field investigations
→ Case development
→ Reporting and action

That integration matters because investigations rarely move in a straight line.

A pattern identified by analytics may require a medical reviewer.

A provider concern may require field validation.

A field observation may send the analytics team back to look for a broader pattern.

The process works best when those capabilities are connected.

Technology should not sit in one silo.

Investigators should not sit in another.

Prevention is getting earlier. Detection is getting smarter. Investigation still determines what’s real.

As comes to a close in Portland, one thing is clear: Medicaid program integrity is continuing to move upstream.

Prepayment review.
Connected data.
Advanced analytics.
AI-informed detection.
Stronger prevention strategies.

All of these tools can help identify potential risk earlier—and that is an important step forward.

But earlier detection does not eliminate the need for investigation.

A flag still has to be validated.
A pattern has to be understood.
Documentation has to be examined.
Connections have to be followed.
Evidence has to support the conclusion.

And ultimately, someone has to determine whether what looks unusual is an error, an emerging scheme, or intentional misconduct.

The strongest program integrity models will not choose between technology and investigative expertise. They will connect them.

That is one of the conversations we’re leaving thinking about most—and one we look forward to continuing well beyond Portland.

Thank you to NAMPI and the Medicaid program integrity community for the conversations, collaboration and shared commitment to protecting critical programs.

Earlier detection is changing Medicaid program integrity.

But detection is not the finish line.

One theme coming through clearly at is the industry's continued move upstream.

-> Prepayment review
-> Advanced analytics
-> AI-informed detection
-> Connected data

All can help Medicaid programs identify potential risk earlier—and that matters.

But as the conversation increasingly shifts toward when we can identify suspicious activity, there is another question program integrity organizations cannot afford to overlook:

What happens after the technology finds it?

-> An anomaly is not automatically fraud.
-> A flagged provider is not automatically a case.
-> An AI-generated lead is not automatically a credible allegation.

- Someone still has to connect the providers, members, claims and entities.
- Someone has to review the documentation.
- Someone has to understand whether the activity represents an error, an emerging scheme or intentional misconduct.
- And someone has to develop the evidence necessary to determine the appropriate next action.

The future of program integrity isn't technology versus investigators. It's technology making experienced investigators better—and experienced investigators making intelligence actionable.

At CoventBridge Integrity Systems, we believe stronger program integrity happens when data, technology, investigative intelligence and real-world evidence work together.

Because finding risk earlier is powerful.

Knowing what to do with it is what protects the program.

Fraud is rarely just one claim, one provider or one data point.

That’s one of the reasons conversations happening this week at matter.

As Medicaid program integrity leaders come together in Portland, there’s increasing focus across the industry on finding risk earlier—through better data, analytics, AI and prepayment strategies.

But identifying a signal is only the beginning.

The harder questions come next:

→ Is it an isolated error—or part of a larger pattern?
→ What other providers, members or entities are connected?
→ Does the documentation and real-world evidence support what the data appears to show?
→ And how do you turn a lead into an actionable investigation?

Technology can help organizations see more, sooner.

Investigative expertise helps determine what those signals actually mean—and what should happen next.

Strong program integrity requires both: the ability to identify risk and the experience to follow it through.

That intersection of intelligence, technology, investigation and real-world evidence is where our CoventBridge Integrity Systems team is proud to support government and healthcare organizations every day.

If you're at NAMPI this week, connect with our team in Portland. We look forward to the conversations ahead!
