Are you organized, compassionate, detail-oriented, and great at keeping things moving? B&E Home Care is looking for a Client Coordinator to take ownership of the client intake process from the moment a referral arrives through the clientโs first 30 days of care.
๐ Why This Role Matters
You will help make sure every new client receives a consistent, professional, and organized onboarding experienceโregardless of payer or referral source.
๐ What Youโll Do
โข Manage new referrals and client intake from start to finish
โข Create client files and ensure required documents are collected
โข Schedule assessments and coordinate with families
โข Verify Medicaid, VA, CareScout, LTC insurance, and private-pay information
โข Enter authorizations, approved hours, and schedules accurately into SwyftOps
โข Coordinate with the RN and Scheduling Coordinator
โข Track authorization renewals and prevent lapses
โข Complete Day 1, Week 1 & Day 30 client follow-ups
โข Document all communication and follow-up in the client record
โข Escalate missing documents, authorization issues, and concerns promptly
โญ Weโre Looking For Someone Who Is:
โ๏ธ Highly organized and detail-oriented
โ๏ธ Compassionate and professional with clients and families
โ๏ธ Strong on the phone and in person
โ๏ธ Able to manage multiple referrals and deadlines
โ๏ธ Comfortable learning technology and CRM/scheduling systems
โ๏ธ Experienced in home care, healthcare admissions, or care coordination preferred
โ๏ธ Familiar with Medicaid, VA, or LTC authorization processes a plus
โ๏ธ Able to pass a Level 2 background screening, as applicable
This is more than an administrative position. You are the person who helps turn every referral into a smooth, properly authorized, well-organized start of care.
