💙 Celebrating Case Management Week at Baptist South! 💙

Last week, Cornerstone Caregiving St. Augustine had the pleasure of hosting an appreciation lunch for the incredible case management team at Baptist South in honor of Case Management Week!

We are so grateful for the dedicated professionals who work tirelessly behind the scenes to advocate for patients, coordinate care, support families, and help ensure safe and successful transitions from the hospital to home.

At Cornerstone Caregiving, we understand that the journey home after a hospital stay can come with challenges, uncertainty, and a need for extra support. We are honored to partner with case managers who share our passion for putting people first and ensuring patients have the resources and care they need to thrive at home.

🐾 The Sweetest Part of Caregiving: Our Furry Friends🐾

Sometimes, during life’s most difficult seasons, the smallest moments can bring the biggest smiles. For many of our Cornerstone Caregiving clients, that special comfort comes from a beloved pet who has been by their side through it all. ❤️

Our caregivers don’t just care for our clients — sometimes, they lend a helping hand to their four-legged family members, too. Whether it’s feeding, letting them outside, giving them a little extra attention, or simply making sure they are cared for when their loved one needs additional support, pet care can be an important part of helping our clients make it through their day-to-day lives.

Because we understand that pets aren’t “just pets.” They are family. They provide companionship, comfort, laughter, and a reason to smile on even the hardest days. 🐶🐱🐾

And, of course, we can’t talk about Cornerstone’s furry family without giving a special shoutout to our very own mascot… sweet little Coconut!🥥🐾 Coconut has a special way of bringing joy wherever she goes and reminding us why the little moments matter so much.

This week, I had the honor of participating in the “Our Veterans Matter” event in Jacksonville alongside Justin Hanson and Bonnie Powell Bedenbaugh. 🇺🇸

It was such a meaningful opportunity to connect with our wonderful referral partners, strengthen relationships within our community, and spend time with those who share our passion for serving others.

I’m incredibly grateful for the opportunity to represent Cornerstone Caregiving and be part of an event that recognizes and honors the service and sacrifice of our veterans. Events like this are a great reminder of the importance of coming together, building meaningful partnerships, and making a difference in the lives of those we serve.

Today was another great opportunity to connect, collaborate, and share what we do at Cornerstone Caregiving! 💙

We had the pleasure of hosting a Lunch & Learn at Encompass Health Rehabilitation Hospital St. Augustine, spending time with an incredible team of case managers who work tirelessly to help patients recover, regain independence, and transition safely back home. After lunch we had a drawing for a “afternoon power basket” and Carmen was super excited to win!

At Cornerstone Caregiving, we believe the care journey doesn’t end when a patient leaves the hospital. That transition home is often when families need the most support, guidance, and reassurance.

We’re passionate about partnering with healthcare professionals to help bridge that gap — providing compassionate, dependable non-medical in-home care that helps seniors remain safe, supported, and comfortable in the place they call home. 🏡💙

Thank you to the Encompass Health team for welcoming us, sharing your time, and for everything you do for the patients and families in our community!
