SAFETY VIOLATION ALERT

This image depicts multiple serious workplace safety violations observed at an industrial/port facility:

Broken WAC (Washington Administrative Code) Violations:

WAC 296-869-60005 – Aerial Work Platforms: Scissor lift platform being elevated via forklift forks — strictly prohibited. Scissor lifts must be used per manufacturer specifications only.

WAC 296-869-60010 – Employees must not be elevated on a platform not designed for the lifting device being used.

WAC 296-800-300 – General fall protection requirements are clearly not being met at elevation.

WAC 296-155-24611 – Prohibition on using forklifts to elevate personnel without a specifically designed and rated personnel basket with full fall protection systems in place.

WAC 296-155-505 – Improper use of powered industrial trucks (forklift) for lifting personnel.

Spot the violations... we'll wait.

This photo is a checklist of what NOT to do on a jobsite:

❌ Ladder not secured at the top or bottom — WAC 296-155-480 & OSHA 1926.1053
❌ No fall protection at height — WAC 296-155-24510 & OSHA 1926 Subpart M
❌ Ladder extends way beyond the landing point without proper tie-off
❌ One guy holding the base of a ladder that's 3+ stories tall — that's not a spotter, that's a witness
❌ No aerial lift, scaffolding, or proper elevated work platform in sight
❌ Wet floor cones are doing more work than the safety plan

Falls are the #1 cause of fatalities in construction. This is exactly the kind of situation WAC and OSHA regulations exist to prevent.

"When the ladder budget runs out but the work doesn't stop."

This is what NOT to do. Let's break down the WAC codes being violated here:

Broken WAC Codes (Washington State):

WAC 296-863-400 — Powered Industrial Trucks Standing on the forks/elevated platform of a forklift is strictly prohibited. Forklifts are not personnel lifts.

WAC 296-874-20005 — Fall Protection Worker is elevated well above 4 feet with zero fall protection — no harness, no guardrails, nothing.

WAC 296-874-10005 — Aerial Work Platforms Using equipment not designed for personnel lifting to elevate a worker violates aerial/elevated work platform standards.

WAC 296-800-160 — PPE Requirements No hard hat, no fall arrest harness visible — required PPE for elevated work is completely absent.

One wrong move equals a catastrophic fall. No job is worth your life. Always use the right equipment for the right task.

If you see this on a jobsite, stop work immediately.

A worker standing on stacked pallets elevated by a forklift is not a solution it is a fatality waiting to happen.

OSHA Violations visible here:
29 CFR 1926.451 - Scaffolding: Pallets are not approved scaffolding material.
29 CFR 1926.453 - Aerial Lifts: Workers must be in an approved, secured aerial lift platform.
29 CFR 1926.501 - Fall Protection: No guardrails, no harness, no compliance.
29 CFR 1910.178 - Powered Industrial Trucks: Forklifts cannot be used to elevate workers on improvised platforms.

Each of these violations carries fines starting at $16,131 per citation. Willful or repeat violations can reach $161,323 per violation — and that does not include the legal costs, project shutdowns, or the human cost of a preventable injury or death.

The most common hazards are also the most preventable. Know the standards. Follow them.
