About Jeff Crume – Founder of Golden Age Transitions

Hi, I’m Jeff Crume. For over 25 years, I’ve had the sacred honor of walking with families through some of life’s most difficult moments—as a hospice chaplain, bereavement counselor, and spiritual care provider. I've sat with families during final breaths, helped sons and daughters navigate impossible choices, and offered peace when everything f

elt uncertain. Golden Age Transitions was born out of that same heart: to serve, simplify, and stand beside families when living alone is no longer safe—and when the next step feels overwhelming. I bring more than experience—I bring presence. I move slowly when others rush. I help families breathe again. My work isn’t about transactions—it’s about transitions. Sacred ones. I now serve families across Arizona and Southern California, helping them explore assisted living, memory care, and personalized senior placement solutions with clarity and compassion. Today, I stood beside the bed of a woman living with end-stage Alzheimer's disease.

Her windowsill was lined with photographs from a life well lived. Family gatherings. Smiling faces. Milestones captured in time.

One photograph stopped me.

It was a picture of her at a retirement celebration. The cake in front of her read:

"Happy Retirement."

I looked from the photograph to the woman lying quietly in her bed.

And I found myself wondering:

"When was the last time she actually knew?"

Not knew facts.

Not knew what day it was.

But knew in the way we often take for granted.

Knew who she was.
Knew the people she loved.
Knew the story those photographs were trying to tell.

It struck me how fragile the things we use to define ourselves can be—our memories, accomplishments, titles, and milestones.

And yet, as I stood there, another thought emerged.

Even when memory fades, dignity does not.

Even when cognition changes, a person's worth does not.

The woman in that bed is no less deserving of tenderness, respect, conversation, laughter, touch, and love than the woman smiling beside her retirement cake years ago.

Perhaps one of the greatest invitations in caring for older adults is learning to see beyond what has been lost and remembering what remains.

A life still worthy of honor.

A story still deserving of reverence.

A person still deeply, immeasurably valuable.

May we never forget that.
