Direct Support Professional

Job description:
Reports to Service Coordinator and/or House Manager
Direct Support Staff have many roles in supporting individuals in many of life's areas at home and in the community. Staff help individuals become more independent, have meaningful lives and reach their life vision.
Must be at 18 years of age or older
Must have a high school diploma or GED
Must pass a national background screening and drug test
Reliable transportation, driver's license valid insurance/registration is required.

Duties and requirements
1. Oversee and assist individuals as needed during meal preparation, personal hygiene, grooming and other activities as needed.
2. Maintain clinical records as designated by policy and procedures such as, but not limited to, documentation of assigned trainings, physical observation, significant observation notes, and any behavioral management programs or outings.
3. Provide input to Meaningful Day activities for each individual.
4. Have a reliable vehicle and cell phone.
5. Provide transportation to residents as required.
6. Supervise individuals in recreational activities both at home and in the community.
7. Be responsible during evening hours to contact appropriate administrative staff in case of an emergency.
8. Make written reports of significant incidents occurring during shift.
9. Serve, as an informal advocate for individuals needs and desires.
10. Supervise individuals on self-medication.
11. Complete required documentation
12. Assist with chores, weekly budgets and special requests, if applicable.
15. Supervise individual during meal preparation.

Caregiving: 1 year (Preferred)
