Deeply saddened to hear of the passing of Professor Karl Weick.

I had the privilege of co-authoring High Reliability for a Highly Unreliable World with Daved van Stralen and Spencer Byrum. We were honored that Professor Weick wrote the foreword for the book.

Re-reading his words today, this passage especially stood out to me:

“The beauty of this volume is that pathways to more reliable functioning are made explicit even if, at times, that explicitness feels daunting... ‘I may not know what is happening, but I know what to do.’”

Thank you, Professor Weick, for your extraordinary contributions and enduring legacy.

SafeMTS recently presented how Large Language Models can improve classification, consistency, and analysis of maritime near-miss reports — enabling faster, actionable safety insights within a secure CIPSEA-protected environment.

We are humbled to contribute to an effort focused on preventing serious events and ultimately saving lives at sea.

Excited to be presenting at the 4th International Maritime Human Factors Symposium on: "Lessons Learned from Applying AI to Near-Miss and Accident Data – Insights from SafeMTS and Beyond"
We’ll share what we’ve learned from applying AI and LLMs to thousands of near-miss and accident reports within SafeMTS and across other maritime & aviation datasets —including how AI improves data quality, uncovers weak signals and causal patterns, and supports proactive safety decisions.
We’ll also discuss practical approaches for:
• Building scalable AI-assisted classification workflows
• Enhancing data completeness and consistency
• Strengthening near-miss programs to prevent future accidents
Looking forward to contributing to the Maritime Safety and Human Factors dialogue.
