Women play an extraordinary role throughout healthcare - as nurses, caregivers, administrators, clinicians, educators, entrepreneurs, advocates and leaders.

But leadership in healthcare is not only about titles. Sometimes it looks like speaking up for a patient, teaching a new team member, helping a frightened family understand what comes next, improving a process, leading with compassion, or creating a solution where one didn’t exist before.

A care plan is important.

But what happens between the plan and the person receiving the help they need?

That is where coordination matters.

Good care coordination can involve understanding the situation, identifying appropriate resources, communicating clearly, activating support and continuing to pay attention as needs change.

For families, that can mean fewer unanswered questions. For healthcare professionals, it can mean greater confidence that the next step is being addressed. And for the individual receiving care, it can mean feeling supported rather than passed from one step to another.

You don’t have to figure it out alone

I want to describe a pattern I've seen more than once.

A patient is discharged.
The medical plan is solid.
The paperwork is complete.

But nobody flagged that they were managing anxiety, depression, or a behavioral health condition that made following through on the care plan nearly impossible.

No mental health follow-up was coordinated.
No family was prepared for what to watch for.
No one owned that piece.

Three weeks later, readmission.

The care plan wasn't wrong.
It was incomplete.

That's the coordination gap I'm talking about.
