A lot of the project management problems we see start the same way: a client tells us the team "doesn't use" whatever system is in place.
When we dig into it, tasks live in the tool. Decisions happen in a hallway conversation or a chat thread. Status updates happen out loud in a meeting, not logged anywhere.
