Quality doesn’t usually fail because people don’t care.
It fails when organizations keep treating symptoms instead of causes.

Root Cause Analysis is how healthy systems pause long enough to ask:
• Why does this keep happening?
• Where is the breakdown really occurring?
• What in the process needs to change — not who?

When RCA is done well, it:
✔️ protects people
✔️ turns data into decisions
✔️ builds learning instead of blame
✔️ strengthens long-term quality health

If the same issues keep showing up, the system is talking.
RCA teaches leaders how to listen.
