Dementia is now the strongest predictor of whether dual-eligible older adults begin long-term services and supports in a nursing home rather than in assisted living or other home- and community-based settings.

A new JAMDA study found that while HCBS use continues to grow nationally, beneficiaries with a prior dementia diagnosis are significantly more likely to initiate care in a nursing home. Other conditions like diabetes and chronic kidney disease also influenced placement decisions.

This raises important questions for providers and policymakers. Are families receiving clear education early enough? Are community-based services equipped to support more complex cognitive needs? Are transitions happening proactively or reactively?
