- Felony background
- Assault and battery
- Domestic violence
- Pending or active restraining orders
In the last 3 - 5 years

If you do have any other case(s) prior that doesn’t include the above cases in the last 3 - 5 years
You will need:
- A copy of your” judgment of conviction”
- A copy of the “Police statement”
- A “person statement”

• A copy of the Police Report and/or Criminal Complaint
Contact the reporting Police Station for Police Report. Contact the processing Court for Criminal Complaint.

• A copy of the Judgement of Conviction (NOTE – CCAP printouts will only be accepted if stamped & signed by Clerk of Court)
Contact the processing Court for Judgment of Conviction. This document shows the verdict or end result of your case.

If you discover the required documents are not available after contacting the appropriate agency/police department, and/or court, please indicate this in a personal statement and submit the personal statement to the Department, along with any documentation that is available and a letter from the appropriate agency/police department, and/or court records custodian confirming the unavailability of the records.
