Do your doctors really talk to each other?
For many patients and families managing complex medical care, the answer is “not enough.” Your story can help change that.
As part of the "Care Coordination Study", I’m leading a national survey to understand how people experience communication among their healthcare provider, what’s working, what’s breaking down, and where care feels fragmented.
Our goal is to identify practical ways to improve collaboration and make care more connected, from the patient’s point of view.
