Are all incidents preventable?

This is a very difficult question and one which has caused many QHSE people hardship as well as management.

As a safety professional I feel bound and sometimes expected to say yes. I do subscribe to H.W. Heinrich's domino theory where he sets out that each incident is one factor in a sequence. Each factor can be visualized as a domino in a series. For one domino to fall it is dependent on the preceding domino to fall in which a chain reaction is started. By removing the root cause domino it will prevent the chain reaction thus diverting the incident.

However, I also believe that incident prevention is based on foreseeable risk to be able to mitigate them. One cannot prevent what one doesn't see is one argument that can be made. In some instances situational awareness can play a big part in mitigation. Couple this with job knowledge, hazard recognition, stop work authority, behavior based safety programs and many others and we seem to be able to think we can prevent all incidents....in theory.

Then the Swiss Cheese Incident Causation Model comes in to play showing us that all the aforementioned systems can be defeated allowing an incident to line up in just the right way allowing it to happen. This of course occurs when systems are not implemented properly, situational awareness is low, complacency has set in and a number of other factors mainly pointing at human error. So another argument has been stated that as long as humans are involved all incidents cannot be prevented.

The problem with Heinrich's theory is that it is based on hindsight to be able to promote future incidents from being repeated. Unfortunately we do not live in hindsight so I have a slightly different take on the original question. My own conclusion is that all incidents are preventable? YES. Can we prevent all incidents? The answer to me has to be NO.

An example of this is when a person has an incident such as a fall. When we put Heinrich's theory to the test we can find the root cause(s) and put mitigations in place to prevent future incidents but that doesn't mitigate the initial incident from happening. Even when mitigations have been put into place such as three point contact, signage, handrails, high visibility paint, non-skid tread, etc. the same incident has still occurred many times over. So each incident investigation is reactionary and based on hindsight. When we can get to a place where we are fully preventing incidents in a proactive manner then all incidents cannot be prevented.

In my opinion there are just too many variables at play to believe that all incidents can be prevented and we cannot protect against what we do not see or know. However, we should strive to eliminate all risk to As Low As Reasonably Practicable and continue our focus on the control of hazards.
