Gravity doesn't negotiate. Handrails do their best work when nobody notices them , which is exactly the problem with this jobsite.

No rail, no guard, no plan B. Just a several-foot drop and a lot of optimism.

OSHA's fall protection standard — 29 CFR 1926.501(b)(1) — requires protection at elevations of 6 feet or more in construction. This one wasn't close to compliant, and more importantly, it wasn't close to safe.

Falls are still the number one cause of death in construction. Not electrocutions, not struck-by incidents...falls. And most of them come from gaps just like this one: temporary conditions that everyone means to fix "before someone's up there again."

If your crew is working near an edge without protection, that's not a checklist item. That's a Tuesday that could go very wrong.

Nothing beats hands-on training when it comes to aerial lift safety.

I had another operator up on the platform and we ran through pre-operation checks, guardrail inspection, and proper positioning before we ever left the ground. This is exactly why classroom-only certification falls short , you can memorize OSHA's scissor lift standard (29 CFR 1926.453) all day, but muscle memory and situational awareness only come from actually operating the equipment under supervision.

Every operator who leaves one of my trainings knows how to inspect the machine, recognize a hazard before it becomes an incident, and operate with confidence, not just pass a test. This is what saves lives , prevents injuries, and keeps your crew safe each day!

On a recent site walk, we found a platform edge with a top rail installed but no midrail underneath it. That gap between the top rail and the working surface is more than enough room for a worker to go through in a fall or a slip toward the edge, ot the rail to give way potentially.

Under OSHA's fall protection standard — 29 CFR 1926.502(b)(2) — a midrail (or an equivalent barrier — screen, mesh, intermediate vertical members) is required between the top rail and the walking surface any time there isn't a wall or parapet at least 21 inches high.

A top rail alone doesn't satisfy the standard, and it doesn't stop a fall the way crews assume it will.

This is a fast, cheap fix compared to what it's protecting against. It's also one of the most common things we flag on inspections, not because crews are careless, but because it's easy to assume "there's a rail" means "it's covered."

Compliance needs to be about caring for our team members, preventing injuries, and saving lives!

Ask most equipment operators how they learned to run a skid steer or a mini excavator, and the answer is almost always the same: someone showed them once, then they were on their own.

That's exactly how tip-overs and pinning injuries happen. NIOSH's research on skid steer fatalities found that most cases involved an operator pinned between the machine and a fixed object — usually because they weren't clear on blind spots, swing radius, or what "safe distance" actually means around moving lift arms. Add uneven terrain or an overhead obstacle, and a routine task turns into an incident report.

The compact excavator and skid steer pictured here are two of the most common machines on job sites across Tennessee... and two of the least formally trained on.

A turnbuckle is only as strong as the pin that keeps it from turning.

This one's tightened up and looks fine at a glance. No cotter pin. No safety wire. No locking nut backed down...nothing physically stopping that barrel from working itself loose under vibration or load cycling.

In this photo we see it is not installed correctly and is missing components. That means this turnbuckle poses a serious fall risk that could cause serious injury or death.

"It's tight right now" isn't the same as "it's installed correctly." Anchorage hardware needs a positive locking method every time, not just snugged down and left.

This is the kind of thing our inspections are built to catch...easy to miss if you're not specifically looking for it.
