Your driver license can be revoked due to multiple DUI convictions.

To have them reinstated, you must complete your suspension period, pay all court fines and reinstatement fees, provide proof of financial responsibility (SR-22), and often complete court-ordered alcohol education/treatmemt.

Also, you must complete the revocation form from the MVD. This requires an evaluation from a licensed counselor.

Beyond the Hurt Counseling Center licensed counselor can assist you with the evaluation then mail off the results.
