“Every discharge plan eventually meets a floor plan.”

That handoff from healthcare back to HOME matters.

A discharge plan can include medications, therapy, follow-up appointments, mobility support, and caregiving. But eventually, all of that has to work inside an actual home with actual doorways, bathrooms, bedrooms, stairs, thresholds, and daily routines.

Here we go… again.

Another article about the challenges of an aging nation, and still barely any discussion about the physical environments people will actually inhabit, or how HOME and PLACE need to change with them if they are going to succeed. UGH.

In 2014, I flew to Osaka, Japan for the International Home Health Expo and visited TOTO’s R&D Department, where I wore an age-suit designed to simulate being 85 in terms of mobility, strength vs. weight, eyesight, hearing, and tactile recognition.

I struggled. I sweated. 😅 But what I learned was empathy, and that makes me a better designer.

At the Expo, I saw innovative solutions for mobility, bathing, caregiving, transportation, and more.

All good progress.

But still in SILOS.

Who is integrating it all?

Who is planning the infrastructure of the HOME itself so it can adapt when a specific need shows up later?

🏡 HOME has to be part of solving the Super-Aging problem.

Whether someone wants to stay there, needs to stay there for financial reasons, has family moving in, needs care, or simply wants more options later, HOME is where much of this is going to happen.

So what are we doing to make HOME ready?

That is the discussion that is needed.
That is the discussion that is missing.

Same person. Same diagnosis. Different house. Different options.

A change in health does not happen in a vacuum. The home can either create more obstacles, or give someone more ways to keep living with independence, dignity, and choice.

Sometimes that means adding support later. Sometimes it means planning for flexibility from the beginning. Either way, the physical environment matters. 🏡

The goal is not to predict every future need. It is to create a home that leaves room for more possibilities when life changes.

A resilient home should be ready for more than the conditions outside. It should also be able to support the people inside as life moves through different chapters.

That might mean a child coming home, a parent moving in, mobility changing, caregiving becoming part of daily life, or simply wanting a home that keeps working as the years go by.
