The CY 2026 Physician Fee Schedule includes a -2.5% efficiency adjustment applied to work RVUs and intra-service physician work time for most non-time-based CPT/HCPCS codes.
For surgical specialties, this may mean practices do not see the full benefit of the 2026 conversion factor increase, creating additional reimbursement pressure on procedures, margins, and site-of-service strategy.
