On March 20, 1989, a cryogenic tank filled with 7,000 tons of ammonia at a fertilizer plant in Lithuania suddenly ruptured and ignited, killing 7 and injuring 57 workers. Do you suffer from the ERS design ostrich syndrome? Most companies are well-equipped to perform emergency relief system (ERS) design for single-phase flow and non-reactive systems. Existing standards and recommended engineering practices developed by organizations such as the ASME, NFPA, API, and AIChE's CCPS enable a competent engineer with proper training to perform such calculations with a high degree of confidence and accuracy.

Reactive systems, however, are more complex, and may be susceptible to fire-induced and process-induced runaway reactions.

On December 16, 2002, at a chemical facility in Chalampé, France, a leak of cyclohexane occurred during a maintenance activity on the expansion loop of the DN50 mm manifold. As the piping was not yet equipped with a device for rapid leak detection, it took 30 hours to determine the cause of the pressure anomaly.

Small companies that don’t exceed the threshold quantity requirements to be covered by the Occupational Safety and Health Administration’s Process Safety Management (PSM) of Highly Hazardous Chemicals Standard are still handling hazardous materials that may pose risks to their employees and the public. However, many of these companies have limited resources and find the idea of implementing an entire PSM program daunting.

For example, a Danvers, MA chemical facility had only 15 employees in 2006. On November 22, 2006, the facility released a flammable v***r that reached an ignition source and exploded. The company was destroyed along with 24 neighboring houses and six businesses. They did not have a written process safety program, and the emergency responders and community were not aware of the hazards at this commercial business.
