On January 7, 2026, a worker was fatally injured while working inside a crawl space at Converse Elementary School in Converse, Texas.

According to OSHAโ€™s investigation, the worker was using a mini-excavator to remove accumulated dirt when they became trapped between the equipment and a concrete beam.

Safety training alone cannot eliminate every hazardโ€”but combined with proper hazard assessment, engineering controls, procedures, supervision, and emergency planning, it can help reduce the risk of serious incidents.
