📣 Caregivers and Families: Do You Know About CalAIM?
Caring for someone at home can be overwhelming—especially when their needs increase, they return home from the hospital or their current support is no longer enough.
If the person you care for has Medi-Cal, they may qualify for additional help through CalAIM.
What is CalAIM?
CalAIM is a California initiative designed to help Medi-Cal members receive more coordinated care and access services that address needs affecting their health and ability to remain safely at home.
Services are coordinated through the member’s Medi-Cal Managed Care Plan, also called an MCP. The MCP is the health plan listed on the member’s Medi-Cal insurance card.
What support may be available?
Depending on the member’s needs, eligibility and health plan, CalAIM services may include:
🏠 Personal care and homemaker assistance
💙 Respite services to provide caregiver relief
🍎 Medically tailored meals
♿ Home modifications that improve safety and accessibility
🏥 Support when transitioning home from a hospital or care facility
🤝 Enhanced Care Management for people with complex health and social needs
🏡 Housing-related and other Community Supports
When should a caregiver or family member ask for help?
It may be time to explore CalAIM when:
✅ The person’s health or ability to care for themselves has changed
✅ They are struggling with bathing, dressing, meals or other daily activities
✅ They have frequent emergency room visits or hospitalizations
✅ They are preparing to return home from a hospital or facility
✅ Their current caregiver is experiencing burnout
✅ They do not have enough support to remain safely at home
✅ They may be at risk of hospitalization or placement in a care facility
✅ Their medical, behavioral health and social needs are difficult to coordinate
What can you do?
Document changes in the person’s health, abilities and daily support needs.
Contact their Medi-Cal health plan and ask about CalAIM Community Supports.
Speak with their doctor, social worker, case manager or care coordinator.
Ask whether the person may also qualify for Enhanced Care Management.
Contact Coast Care if you need help understanding where to begin.
Eligibility and referral requirements vary by service and health plan. Not everyone will qualify for every service, but caregivers and families should know that these resources may be available.
At Coast Care, we exist to connect people with care—and that starts by helping families understand their options.
