4 months.

That is how long a global-period billing error sat on a new client's account before anyone saw it.

It had been made before we took over. We found it, and we fought for every dollar of it.

But by then it was too late. The revenue had to be written off.

That one stayed with us. It is part of why we built AHR's revenue intelligence platform, so our team tracks claim activity daily and gaps like this surface sooner.

What could be slipping through at your practice right now?

One of our clients now has a 1.90% denial rate.

It did not happen by accident.

It started with a deep dive during onboarding, so claims were already getting cleaned up before we even started working them.

Most practices never get that kind of visibility. They find out about revenue leaks after the denials pile up and the A/R gets old.

That is why we built a free Revenue Leak Calculator.

2 minutes. It shows where your revenue may be slipping across denials, underpayments, aged A/R, and coding.

Try it here: ahrevenue.com/revenue-leak-calculator

PS: If you are selecting "I don't know" on any of the questions, you are already in trouble.

190 new diagnosis codes. All live since October 1.

Here's what changed in some of the specialties we work in:

OB/GYN: ectopic pregnancy coding is far more specific, and there's a new category for pregnancies that continue after a vanishing twin.

Orthopedics: new codes for plantar fasciitis, and osteomyelitis now requires laterality.

Primary care: more detail for low BMI, and dilated cardiomyopathy now splits into more specific options. The old general code is no longer billable.

A pain management practice we spoke too had a claim history that constantly showed: “pending.”

Some of those claims had been pending for months. When we asked for their aged A/R, nobody could give us a number. “They’re still processing,” we were told.

So we loaded every claim into our revenue intelligence platform. It ranked which claims to work first, and our team checked each status one by one.

What surfaced: claims stuck in limbo, not processing, just sitting and waiting on a follow-up nobody had made.

“Pending” in your EHR doesn’t always mean pending with the payer, especially when your EHR doesn’t communicate well with your clearinghouse.

If you can’t say what your aged A/R is today, an audit is where we’d start.

Denied.

Appealed.

Approved.

A recent HHS-OIG review found that Medicare Advantage plans overturned 95% of appealed prior-authorization denials for skilled nursing facility admissions.

Yet only 18% were appealed.

The lesson for practices: don’t just track denials.

Track the payer, denial reason, appeal outcome, and days to approval.

Your denial data may reveal which decisions should be challenged.

$25,000.

That's what one psychiatry practice had sitting in denied TMS claims which were stuck with a government payer, & no updates for over 30 days.

This is exactly the kind of claim our revenue intelligence platform is built to catch, before it crosses the 14-to-21-day mark where a claim quietly starts to go cold.

Once the platform flags how long these had gone untouched, our team goes straight to the payer, for a resolution.

Technology caught it and our team is what actually got it resolved.

If your TMS or behavioral health claims are sitting in limbo right now, that's exactly the kind of case we take on.

90 days later.

A behavioral health group’s TRICARE-West application was still pending.

We had planned around a 90-day estimate. The process took longer.

Meanwhile, the group still had patients to serve and a business to run.

That experience reinforced how we already approach enrollment planning: start early, track each milestone, and leave room for payer delays.

For new practices, that means:

- Confirming receipt and outstanding requirements.
- Tracking credentialing, contracting, and effective dates separately.
- Building flexibility into opening, hiring, and lease plans.
- Offering hybrid model until finalization

Submitting an application is a milestone, but it’s not confirmation that your practice is contracted.

A primary care practice came to us convinced their billing was airtight.

It wasn't.

Modifier 25 was being left off visits that qualified. Annual wellness visits were getting billed like regular check-ups.

And chronic care management was rendered but never billed.

None of it was intentional. it's just what happens when billing isn't built around primary care's specific codes.

Modifier 25, AWVs, CCM/PCM is exactly where we come in.

If you're a small or mid-sized primary care practice and you're not sure you're capturing all of it, that's what a free 30-minute revenue cycle review is for.

Clean revenue cycles don't happen

by accident. They're built,

one quarter at a time.

Here's what Q3 looked like for us:

Recovered claims that had been rejected, kept denials minimal across our client base. Submitted claims in under 24 hours of note completion consistently.

Most of the practices we work with aren't massive. They're small, growing practices building their revenue cycle from the ground up

and getting it clean from day one matters more than fixing it later.

That's been the real story of this quarter: not chasing problems, but preventing them before they start.

Same focus heading into Q4.

$300K-$500K.

That's what most primary care practices leave unbilled every year in chronic care management alone.

Here are 5 underused revenue opportunities:

Chronic care coordination: most practices have hundreds of qualifying Medicare patients going unbilled.

Post-discharge follow-up care: high-value, frequently missed.

Bundled care management programs: newer options that simplify billing across multiple care types.

Annual wellness visits: separate from a regular physical, and often skipped entirely.

Ongoing care complexity recognition: ongoing primary care work that quietly goes uncompensated.

Your patients already qualify. The revenue is already there.
