Without collective accountability, many organizations develop a culture that is driven by delivering individual outputs. Individuals focus on what they are rewarded for without sight into the effect on the organizations' purpose and bottom line.

Have you heard of individuals rewarded for:
How many lines of code they wrote
How many features they completed
Documents approved
Individuals trained

The list goes on, and without understanding the impact of these outputs on moving towards the organization's purpose, we are reinforcing a silo mentality.
