C9290 Injection, bupivicaine liposome, 1 mg
C9291 Injection, aflibercept, 2 mg vial
C9733 Non-ophthalmic fluorescent vascular angiography

Effective April 1, 2012, CMS is implementing logic changes to the OCE to ensure that separate payment is made for skin substitutes only when they are billed with a skin substitute application procedure. In other words this is a “biological to procedure edit”.

Received a notice from CMS yesterday (3/15/12) that MACs have not been paying for infusion services (96365 and 96366) in conjunction with the provision of Provenge®, HCPCS Q2043, due to a bundling edit. CMS has instructed the MACs to adjust claims for DOS on or after 7/1/11 containing charges for infusion that were denied and not paid WHEN BROUGHT TO THEIR ATTENTION.
If you have this HCPCS code in your CDM we recommend you run a report to identify all claims reported with this HCPCS code and rebill with the appropriate infusion code(s) (96365, 96366). Claims cannot be rebilled to your MAC until 4/1/12 and so you should run this report on or after 3/31/12.
