Support Coordination
Support Coordination (formerly case management) is defined as services provided to Medicaid eligible individuals to assist them in gaining access to needed waiver and other State Plan services as well as needed medical, social, educational and other support services within the Office for Citizens with Developmental Disabilities (OCDD). This would include the Children’s Choice Waiver, Supports Waiver, Residential Options Waiver and New Opportunities Waiver. Support Coordination will develop quality person centered plans, complete necessary tasks to ensure plan approvals and revisions are timely, assist in the development and coordination of community resources, and monitor plan implementation to assure personal outcomes are met. Support coordinators will provide information and assistance to waiver individuals in directing and managing their services under the self-direction option.
