It’s Molly Bolnick here and today I’m debuting something special I've been working on for our community in STC. The St. Charles County Senior & Family Resource Guide!

A while back I asked some local community members for their input, and I was honestly overwhelmed by all the incredible suggestions. Home care, hospice, senior living, healthcare, transportation, elder law, real estate, personal services, and so much more.

I wanted to compile everything into one guide so it's easy to save, print, or pass along to someone who needs it. If you are navigating this season of life, I’ve got you covered! Huge thank you to everyone who contributed. This community is something special and I’m proud to be a part of it.

I'm Molly Bolnick a nurse care manager and dementia educator at Woven and I am so excited to start this Dementia Education Series at Immaculate Conception of Dardenne Catholic Church! Dementia Life STL

Whether you're caring for a spouse, parent, friend, or simply want to better understand dementia, I'd love for you to join us.

Our first session, Dementia 101: Understanding the Basics for Better Care, is Monday, September 21st at 4:00 PM. We'll cover the basics of dementia, how it differs from normal aging, and simple, compassionate strategies you can use right away. It's free, no RSVP is needed, and everyone is welcome.

Stories from a Home Care Manager – My name is Kathryn Kearney Foy and I want to share a bit of my story — because honestly, I think it's kind of funny how life works out.

Upon graduating from PT school, I was fully intent on specializing as a sports orthopedic physical therapist. My goal of supporting athletes through similar challenges was inspired by my own experience as a swimmer at Northwestern, when a back injury threatened to cut my career short and a dedicated PT guided my recovery.

Roughly two years into my practice, my boss at the sports ortho clinic suggested I assist on weekends with his home health PT business. Recognizing the heavy burden of my student loans, he proposed this as an opportunity to generate extra income. My very first thought — and I am not proud of this — was absolutely not. I am NOT working with “old people”.

It took exactly two weekends.

Two weekends, and I went back to him and asked if I could make the switch to full time home health PT. Something about working with older adults struck a chord in me that my previous work simply wasn't reaching and I have never looked back.

I became a certified geriatric specialist PT in 2015, and what I can tell you is that this population has given me far more than I have ever given them. Every single day I learned something new — from my patients, from their families, from colleagues who are just as passionate about protecting and supporting older adults as I am.

I had absolutely no idea that something I was doing begrudgingly to pay off student loans would turn out to be the greatest gift of my career.

I have worked closely with this population for 26 straight years of my career. While I no longer do direct home health PT, those years laid the most serendipitous foundation for the work I do at Woven. For a long time, I have witnessed the clear need in this field firsthand — and I am so proud to be part of an organization that is truly passionate about meeting those care needs in a dignified way.

And on a personal note — I am a card-carrying member of the sandwich generation myself. So I get it. Not just professionally, but personally. I’ll be sharing tips and tricks to make your life easier, simple exercises to keep you mobile, providing resources and support.

If you caught our post last week, you already know we’re growing! But today, we want to share a little more personally about who Lyndsey is and why she is so deeply passionate about our mission at Woven Home Care.

Lyndsey is a nurse with 15 years of experience, but her connection to this work goes far beyond her professional background. For Lyndsey, the belief in what we do is deeply personal. After helping her own family through tough health journeys—navigating hospice care with her mother and watching her stepdad manage a solitary recovery after multiple surgeries—she knows exactly how much a supportive, extra layer of assistance can mean to a family. Whether it's help with meal prep, laundry, or simply knowing someone is there to ensure a loved one is safe, she truly understands the impact of what we do.

We are so incredibly lucky to have her leading the charge to bring compassionate care to the Des Moines community.

Here is a little bit more about this wonderful leader.

Lyndsey Schwanz brings over 15 years of nursing experience and progressive leadership in healthcare, with a strong focus on quality, patient safety, and clinical effectiveness. She is a Registered Nurse (RN) with a Master’s degree in Nursing Leadership and Administration, and holds certifications as a Certified Professional in Healthcare Quality (CPHQ) and a Certified Professional in Patient Safety (CPPS). Lyndsey is deeply committed to enhancing home care by ensuring each individual receives safe, personalized, and high-quality support in the comfort of their own home. Her work is guided by compassion, intentionality, and a drive to make meaningful impact.

Lyndsey is honored to serve as Regional Lead – Care Manager for the Des Moines market, Lyndsey is proud to support Woven's expansion into Iowa. She is passionate about helping individuals remain in their homes as long as possible while ensuring they receive the safest and most effective care.

Outside of work, Lyndsey enjoys spending time with her husband and friends, and cheering on her three children at their sporting events. The family’s goldendoodle, Millie, brings an extra dose of joy to their home.
