STRATEGIC PLANNING FOR ONCOLOGY SERVICES - Part 1: Common Challenges

Strategic planning is often a frustrating activity, arguably never more so than for oncology. So many departments are involved in oncology care and often those departments report to leaders who are not involved in or familiar with oncology. It can be extremely difficult to get those leaders and departments to participate in the planning, and if they do not participate, it is far less likely that they will do what is necessary in order for the oncology strategies to be implemented.

One example might be an initiative to develop a screening program that impacts on radiology can be derailed if radiology does not support additional equipment, staffing, or changes to how the radiology department staff and physicians operate. Similarly, a strategic initiative to develop a true breast center that would not only impact on mammography that falls under radiology could die on the vine for the same reasons. In addition, that breast center initiative might call for physicians to function differently and if they could resist if they didn’t buy in to the value of the concept.

Additional examples include support services and physicians. When patient support services report centrally, it can be impossible to implement initiatives that call for new staff, new skills, or new time commitments. If physician care is provided by physicians who are not committed to the service line, their willingness to spend the time and energy to implement a second opinion or multi-disciplinary clinic may not be forthcoming.

Further challenges may come if senior leadership does not understand some of the external issues that drive oncology-specific initiatives. For example, new treatment approaches can have a substantial impact on physician and non-physician staffing, care management, patient support services, major capital needs, facility, and more. If the C-suite does not have a good understanding of the clinical advances taking place in the world, it will be nearly impossible to gain the support and approval to acquire new technology or to ask other departments to change.

How can an oncology executive mitigate these challenges? That will depend to some degree on the institution’s culture. In the best of all worlds, gathering a planning team that includes individuals who will champion the plan with their respective colleagues is the best bet. When the plan is developed by such a team, there will be buy-in for the resultant strategic initiatives. And there will be physician and administrative leaders who will advocate for implementation of the initiatives with their peers.

Memorial Day 2016- A Personal Tribute to Those That Have Served and to Those That Currently Serve

In every instance, the American soldier has responded admirably when called upon to preserve the rights of this nation and to protect the many freedoms that we value. From the American Revolution when we stood (mostly) alone, through Vietnam- a time of turmoil among the people of our nation, and even today when conflict in the Middle East and terrorism at home and abroad incite similar confusion and disparity among us. So it is of paramount importance, now more than ever, that all Americans demonstrate our respect for the soldiers who fought - and for those who currently fight - and to remember the significant personal sacrifices made by each of them and their families. It is their dedication to service and to country that will ensure that every call to duty is answered with the promise of victory over the myriad threats to our freedoms. They have earned our heartfelt thanks for their commitment to service and our profound and enduring respect!

On a very personal note, I would like to salute the five active-duty service members of which I am so especially proud…all nieces and nephews:

I saw this on the Healthcare Executives Network of LinkedIn and wanted to share! It is a concise and very well-written explanation of chemotherapy treatments, radiation therapy and surgery options for cancer patients. Obviously it would also be a great primer for new staff members in your oncology program or practice. Part 2 (coming soon) apparently will cover stem cell therapy, targeted therapies, and immunotherapy!

The 2016 National Hospital Oncology Benchmark Study (NHOBS) is open to all hospital-based infusion and radiation oncology centers. There is no fee and all participants receive a copy of the Full Report. Please share this information with your professional colleagues at other institutions and appropriate colleagues at your own center.

The registration for the 2016 NHOBS (based on 2015 data) is open...and easy! All hospital-based infusion services and radiation oncology centers are eligible. Following your registration, we will send the submission forms to your email address...we ask that you please review them carefully. We will be scheduling two open-line dial-in webinars (dates to be announced shortly) to get all questions answered for all participants. Following those calls, you will be able to contact us if you have any lingering questions prior to data submission in order to avoid unnecessary work.

Oncology providers… Potential huge cuts from CMS for drug payments!

If you haven’t seen this yet, you need to go look now! CMS proposed this week to pilot an alternative means of paying for outpatient drugs that are paid using the current Average Sales Price (ASP) plus 6%. In a nutshell:

• CMS will select one or more geographic areas for the pilot.

• In the geographic area, providers (physician offices and hospital outpatient infusion departments) will be assigned to one of four groups.
-- Three “test” arms, one for the new ASP model, one for a new Value Based Purchasing requirement, and one for both.
-- A control arm (no change to the current model).
• The new ASP model would reduce payment for drugs to ASP + 2.5% and add a flat fee of $16.80 per drug per day.
• This will not be optional for providers. If you are selected, you will participate.

Will this cover your costs?!

Is the Secretary of HHS really empowered to waive the ASP + 6% model that was established by legislative statute in the late 1990’s?
