Connecticut SNFs are starting to ask the right questions about the PDPM Medicaid transition.

Many facilities understand that PDPM is coming…
but fewer have stepped back to evaluate whether their current processes will actually perform well under it.
That sparked a great discussion with attendees around questions like:
How strong is your interview strategy? Are tools like the PHQ-2/9 consistently captured and reflected in the record?
Do you have a Restorative Nursing Program in place? What could the ROI look like for your facility under PDPM?
Do you have a standing Medicaid CMI meeting?
Who participates?
What’s the agenda?
How are changes communicated across the IDT?
How are inactive or outdated ICD-10 codes being resolved?
Do your ADR and denial processes actually work when they’re tested?
And an important one: does your documentation clearly reflect the care your team is providing every day?
These are the operational questions that will shape how facilities perform as Connecticut transitions to PDPM Medicaid reimbursement.
