Consultation

Senior life transitions are usually a communal effort. For our first meeting, we bring the entire family together as a team to identify challenges and determine solutions. We focus on getting to know you, your loved ones, and your unique situation. That allows us to craft a plan that meets your goals concerning health, safety, and independence. Life Care Management

A comprehensive plan provides daily personal care and activities for the older adult. Whether the care is short-term or long-term, we work closely with family and other providers to ensure a safe environment, preserve dignity and independence, maintain healthy nutrition, and promote mental and physical stimulation. We help navigate the often bewildering maze of housing and medical options, from assisted living to skilled nursing, offering recommendations and assistance. Life Transition Counseling

The aging process tends to sneak up on us. You start to forget everyday things. Tasks once accomplished with ease are now difficult. Decline in physical and mental health in addition to the loss of purpose in life can be overwhelming. It can seem as if the world you once knew is disappearing. The end of someone’s life is a challenging time for the individual as well as their relatives. We specialize in helping you navigate through the hospice process so your loved one can depart this life with dignity and peace. Bereavement Counseling

Lest you feel like that poor fellow, the best direction to take when trying to determine what to do when someone is showing signs of cognitive decline or dementia is to get them to see their primary physician. What happens next depends on what the physician says. Getting them there can sometimes require supreme delicacy, but it must be done.
