Joint Commission survey preparation doesn't start the week before the surveyor walks in. It's a year-round process — and these are the three areas where I see the most facilities fall short.

1. Environment of Care documentation. Ligature risk assessments, emergency preparedness plans, and fire drill logs need to be current, completed, and accessible. Surveyors look for patterns of documentation, not just the document.

2. Staff files and credentialing. Incomplete staff records are one of the most cited findings in Joint Commission surveys. Every file should include current licenses, background checks, competency evaluations, and required training certificates.

3. Performance Improvement and QAPI. Facilities often have a QAPI program on paper, but can't show evidence of how findings are acted upon. Surveyors want to see the loop closed — problem identified, action taken, outcome measured.

3 of the most common DHCS licensing mistakes I see — and how to avoid them.

After 23+ years working inside the addiction treatment industry, I've reviewed hundreds of licensing applications. The same issues come up again and again.

1. Incomplete paperwork. A missing signature, outdated form, or wrong version of a document can stall your application by weeks. Create a submission checklist and verify every page before you send.

2. Policies that don't match your actual program. DHCS reviewers look for alignment between your written policies and your level of care. Generic templates almost always create gaps. Your policies need to reflect how YOUR program actually operates.

3. Facility deficiencies caught late. Don't wait for the site visit to discover your facility doesn't meet the square footage, safety, or ADA requirements for your intended level of care. Walk through the space against the DHCS standards early and often.
